Patients use the Vivify Health mobile app to follow personalized care pathways, share biometric data, and access tailored educational resources and can conduct video visits with their care team. In addition, patients using +Go Mobile App can send and receive secure in-app text messages.
Providers enroll patients either at the healthcare facility or by shipping Bluetooth-enabled remote monitoring devices directly to the patient’s home. Supported devices include blood pressure monitors, pulse oximeters, weight scales, glucose meters, pedometers, and thermometers, with additional devices available as needed.
The app connects to the compatible Bluetooth-enabled medical devices for automatic biometric data collection. However, biometric devices are optional; patients can manually enter their biometric readings, and depending on their program, biometrics may not be required at all.
